Fair Haven Community Health Center (FHCHC)

Since its beginning as a community-initiated free clinic in 1971, the Fair Haven Community Health Center has served a multi-ethnic community located in the eastern portion of the city of New Haven. FHCHC operates within a designated Medical Underservice Area and a Health Professional Shortage Area. A licensed outpatient facility, JCAHO accredited and a federally qualified health center, FHCHC is the only primary health provider in the area—offering a full range of services for all ages. In addition to primary care, the Center provides behavioral health, social services, lab, and WIC services and is affiliated with the Yale-New Haven Hospital and the Hospital of St. Raphael for hospitalizations and specialty and diagnostic referrals. Additionally, community-based comprehensive care is provided at five school-based health centers and a satellite clinic for elderly patients.
